Community Support Team (CST) Case Manager

GENERAL NATURE OF JOB DUTIES
Under broad supervision the Community Support Team (CST) Case Manager will carry provide Case Management to individuals within the CST program. They will ensure compliance with all Bridge Health and the Department of Behavioral Health and Developmental Disabilities (DBHDD) policies and regulations while fulfilling their role. This is a full-time position. Flexibility of schedule is nice to have, primary work schedule will be Monday- Friday 8:30-5:00.
MINIMUM QUALIFICATIONS
Bachelor' Degree and at least 21 years of age, 3 years of good driving history.
Preferred Qualifications: Experience working with individuals who have severe and persistent (SPMI) mental illness.
