Case Manager - Recovery YAS

$21.76 - $28.74 per hour

Must be available to work one of the following shift:

Sun 12-8, Mon & Tue 8-4, Wed & Thu 9-5

Mon 12-8, Tue - Fri 8-4

Position Summary:

Assist clients in developing and achieving vocational and educational goals to their fullest potential, as well as help clients rehabilitate daily social activities for reentry and integration back into social environments.

Duties and Responsibilities:

  • Provide vocational assessment/testing, support and counseling.
  • Promote attendance to rehab activities and functions to ensure social inclusion and interaction among peers in the agency as well as other YAS programs in the state.
  • Provide transportation to clients to interviews and social rehab events.
  • Facilitate vocational, daily living activity and psycho-social groups.
  • Develop and maintain community resources and relationships.
  • Collaborate with treatment team regarding individual vocational/educational/social plans.
  • Maintain client records.
  • Perform any other department or agency-related duties or special projects as directed by supervisor.

Qualifications:

Musical Knowledge preferred.

Bachelor's degree required, with a major in human services/related field preferred.

Ability to work a flexible schedule.

Knowledge of behavioral health and employment/vocational issues.

Good communication and documentation skills.

Computer literacy.

Ability to prioritize and organize schedules/demands.

CPR/First Aid Certified or willing to become certified.

Valid driver's license and access to reliable transportation on a consistent basis.
