JOB DESCRIPTION
POSITION TITLE: CDPA Facilitation Specialist
POSITION DESCRIPTION: BRIDGES operates a variety of programs guided by the philosophy of Independent Living to provide services and supports for individuals with disabilities so that they may experience autonomy in all areas of living. The CDPAP Facilitation Specialist maintains a minimum caseload of 175 to 200 consumers who are individuals with disabilities and their Personal Assistants to successfully meet the requirements associated with CDPAP.
- Associates Degree with a minimum of 2 years' experience in Human Services; or a minimum of 5 years' experience in Human Services
- Provides person-centered, need-based program education and guidance to consumers and authorized representatives specific to individual choices, goals, and desired outcomes.
- Builds relationships to support a positive consumer and personal assistant experience.
- Ensure proper review and submission of all required documentation for timely enrollment completion.
- Maintains documentation of all encounters with consumers, personal assistants and informal supports in accordance with applicable policies and procedures.
- Ensure time and attendance compliance by monitoring usage and providing education where needed.
- Respond to all inquiries in a timely manner and promptly address all questions and concerns as needed.
- Responsible for learning program policies to ensure ability to respond to customer service inquiries.
- Attends all required trainings and in-service programs
- Additional duties as assigned
- Bilingual/ Spanish a plus
- Familiarity with the Independent Living Philosophy and Consumer Directed Personal Assistance Program is a plus.
- Capable of communicating with and in a manner that is culturally sensitive to individuals with disabilities
- Must be able to travel locally and regionally for meetings and enrollments when needed
- Ability to provide exemplary customer service
- Working knowledge of Microsoft Office Software, including Excel, Word and Power Point.
- Ability to become proficient on other software products as utilized by the position
- Excellent interpersonal, organizational, written and oral communication skills; able to interact with all levels of associates in a respectful, confidential manner
- Effective and collaborative team player
- Must be cleared and maintain an acceptable record under the Medicaid Fraud & Abuse exclusion check
- Maintain strong work ethic and strict adherence to code of ethics, standards of conduct and confidentiality requirements.
- All responsibilities for this position should be carried out with the Mission and Goals of the agency in mind; including interactions with people who come to us for assistance, members of the public, representatives from other agencies and county offices, as well as with co-workers in daily office functioning.
- Attend professional development (webinars, workshops, conferences, etc.) to advance knowledge on related programs/activities of the organization as requested by the employee or the Director.
Job Type: Full-time
Pay: $18.00 - $20.00 per hour
