Job Description:
The successful candidate will be responsible for delivering efficient and effective nursing care while achieving positive clinical outcomes and patient, resident and family satisfaction.
Main Responsibilities:
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Assess patient needs and develop strategies to promote independence and improve quality of life.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
Requirements:
To be considered for this role, applicants must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing or awaiting licensure number. Experience in rehabilitative or geriatric nursing is preferred.
