About the Role:
This is an excellent opportunity for a skilled nurse to take on a leadership role in our rehabilitation unit. You will be responsible for assessing patient needs, developing individualized care plans, and collaborating with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
Key Responsibilities:
- Perform comprehensive nursing assessments to identify patient needs and develop strategies to promote independence and improve quality of life.
- Develop and implement individualized care plans that meet the unique needs of each patient.

Qualifications:
To be considered for this role, applicants must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing or awaiting licensure number. Experience in rehabilitative or geriatric nursing is preferred.
