What are the responsibilities and job description for the Wound Care RN Monday-Friday position at Bridgewater Center for Rehabilitation and Nursing?
Bridgewater Center for Rehabilitation and Nursing Center is a skilled nursing facility located in Binghamton, NY. We are currently looking for an outgoing RN to join our wound care team!
Responsibilities :
- Assist in the oversight the facility's wound prevention and management program
- Assess, measure and treat wounds as needed.
- Communicate changes in residents’ conditions to the interdisciplinary care plan team and other partners as appropriate
- Attend Quality Assurance meeting and report on applicable information
- Attend and prepare for weekly Case Management meetings
- Ability to educate staff by conducting in-services and one on one learning
- Ability to investigate, track and trend identified areas of opportunities
- Develop action plans for areas of needed improvement
- Provide partner support and coaching
