MUST HAVE CPR CERTIFICATE, NATIONAL FEDERATION CONCUSSION TRAINING COURSE CERTIFICATE, A SIGNED ANTI-HAZING TRAINING AFFIDAVIT AND A CLEARED CORI (CRIMINAL OFFENDER RECORD INFORMATION).
