POSITION SUMMARY:
Bridgeway Center, Inc. (BCI) is seeking applicants that want to work with clients to effectively address clients' care by assessing needs, developing, monitoring, and facilitating interdisciplinary approaches. This position is responsible for managing an assigned caseload of clients having mental illness diagnosis and for assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.
The Behavioral Health Forensic Case Manager (BHFCM) will provide team case management to adults with serious and persistent mental illness to assist the recipient to remain in the community and avoid institutional care. BHFCM, also known as Intensive team case managers, coordinate needs assessment, services planning, and provide service oversight.
The Case Management department provides services that help individuals link to needed medical, social, education, psychiatric, psychological, and other services. This includes direct assistance in accessing services, Coordination of Care (CoC), oversight of the entire case, and linkage to appropriate services. Case Management includes (1) assessment of the eligible client to determine service needs; (2) development of an individualized, integrated service plan; (3) referral and related activities to help the client obtain needed services; (4) monitoring and follow-up; and (5) evaluation. The Case Management department educates persons served and their support system regarding diagnosis, treatment, and recovery. This position will work with clients in the Judicial system.

MINIMUM REQUIREMENTS:
· Bachelor’s degree from an accredited university or college with a major in Counseling, Social Work, Psychology, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).
· Applicant must be eligible to apply for and obtain Behavioral Health Case Manager certification as provided by the Florida Certification Board (FCB) within one (1) year of obtaining Provisional certification.
o Within thirty days of hire, complete training requirements and apply for Provisional certification.
Bilingual/bicultural English and Spanish preferred, but not required.

Applicant must be at least twenty-one (21) years old and have a valid Florida Driver’s License with no more than three (3) points on their driving record to be insured under BCI’s automobile insurance policy.

DUTIES / RESPONSIBILITIES:
Some of the job tasks and responsibilities include, but are not limited to:
· Follows the Medicaid Handbook in providing a thorough assessment of need, using appropriate releases of information, that includes:
· History is relevant to mental health, representative payee, family’s assessment of the situation, relationship issues, and strengths.
· Psychiatric and medical history describing symptoms, previous diagnoses and hospitalizations, past/current treatment, what was successful and what side effects of medications, medications were not, prescribed, current services, and barriers to treatment.
· Current and potential strengths using the Strengths and Discovery Tool, available resources, support systems, school/vocational placement and progress, employment history, skills, finances.
· Justification of the need for Case Management by identifying needs with mental health maintenance, abstinence from substances, family support, educational/vocational needs, medical/dental needs, legal assistance, increasing support systems.
· At least 1 home visit is completed during the assessment process and monthly thereafter.
· Uses the assessment to develop an appropriate service plan with the person served while using SMART principles (Specific, Measurable, Attainable, Relevant, Time bound).
· The initial service plan is collaboratively completed with the client within 30 days of initiation of services.
· The service plan has measurable short- and long-term goals for the person being served and is relevant to their mental health needs as evidenced by the narrative assessment and service plan.
· Ensures the development of the service plan is done in partnership with the person being served as evidence by documentation in the progress notes and signatures on the service plan.
· A copy of the service plan is provided to the person service as evidenced by documentation in the progress note.
· Service Plan is signed and dated by the recipient, the recipient’s parent, guardian or legal custodian.
· Recognizes changes to client situation and updates the service plan accordingly.
· Assures that all interactions are written and spoken in terms that the client will use and understand.
· If the person being served discloses, he/she is Hard of Hearing or Deaf, the CM will immediately ensure effective communication through the requested aids and complete all relative DCF documents timely.
· Educates clients and their support system to be aware of any signs that may be dangerous.
· Provides flexibility to respond to client needs.
· Educates clients and supports a system of alternative options to hospitalization or a higher level of care.
· Intensifies the level of service and support to clients who have frequent admissions to prevent hospitalizations. Identifies High Utilizer status appropriately.
· Is knowledgeable of community resources and links the person served to the most appropriate service.
· Advocates for the best interest and wishes of the client among the many services providers, agencies, family members and significant others.
· Obtains at least 3 customer service satisfaction surveys per month that meet or exceed the target goal of 85% rating for positive customer experience in collected surveys.
· The following are the productivity standards:
§ During 90 days of employment new Associates must demonstrate increasing levels of billable services delivered each month until they attain a minimum of 120 service hours provided monthly.
§ Associates who have been employed for more than 90 days must maintain a minimum level of billable services delivered each month at 120 service hours.
· Progress notes are completed in the EHR no later than 48 hours after service is delivered.
· Service Plans are up to date and have not expired.
· Assures that progress notes and services provided relate directly to the Service Plan.
· Assures that releases of information are completed thoroughly by all clients/guardians.
· Evidence exists that work is accurate, thorough, neat and completed within required timelines.
· Demonstrates pride in work product.
· Works independently seeking out and taking on new duties; consults with subject matter experts and supervisor to enhance knowledge

WORK LOCATION: In the community work with travel between Fort Walton Beach (FWB) and Crestview (CV). In addition, travel to Florida State Hospital in Chattahoochee, FL.
WORK HOURS: Monday – Friday; 8:00 a.m. – 5:00 p.m.
WAGE: The wage for this position is $20.00 per hour.
