The Community Liaison compensation package includes a base salary and monthly bonus plan. The Community Liaison develops and implements physician and referral source education on home care services, and advances community educational activities on behalf of the company. This position requires strong interpersonal skills, the ability to set and accomplish goals and the ability to respond to competitive marketplace practices.Develops a strategic plan with clear goals and objectives for the healthcare marketEstablishes rapport with physicians' offices to facilitate communication between home health agency and staffIdentifies and promotes agency services to previous and existing referral sources to warrant continued referralsDevelops working relationships with discharge planning and social service department at area facilitiesDistributes medical documents / orders to physicians within established time limitsDevelops and implements education plans for specialty programs for physicians and facilitiesMonitors the effectiveness of plans and education efforts through weekly / monthly reportsPromotes the organization through community service programs i.e. health fairs, health screening programs, and in-service educationPlans for expressions of sympathy, birthday cards, and gifts for special occasions within the healthcare communityMaintains productivity of no less than 35 referral contacts per weekAchieves monthly Personal Production Goals / PPG Requirements Registered Nurse / RN or Licensed Practical Nurse / LPN with experience in social work or education preferred; Hospital Discharge Planner experience as a nurse, social worker, occupational therapist, physical therapist, case manager experience preferredHigh school diploma with a minimum of two (2) years of experience in sales; healthcare, home health, hospice marketing experience preferredCurrent driver's license and automobile liability insurance80% travel / 20% agency office Salary Description $65K to $80K
