Care Plan Coordinator
Position Overview: The Care Plan Coordinator (CPC) plays a vital role in ensuring the effective and personalized care of residents by leading the development, implementation, and ongoing monitoring of their individual Care Plans. This position requires collaboration with an interdisciplinary team, including residents, families, and healthcare professionals, to gather essential information, create tailored care strategies, and ensure the highest quality of care is consistently delivered. The CPC will also be responsible for maintaining comprehensive assessments and MDS documentation, tracking progress, and making necessary adjustments to care plans.
Key Responsibilities:
- Complete the Minimum Data Set (MDS) assessments, including assessment protocols and comprehensive evaluations, in a timely and accurate manner.
- Collaborate with the interdisciplinary team, including residents and their families, to create individualized Care Plans that address specific needs and goals.
- Communicate and educate staff on the developed Care Plan to ensure proper implementation and adherence.
- Monitor the daily care of residents, ensuring that the Care Plan is followed and making updates as necessary.
- Maintain accurate records of each resident's progress, making adjustments to the plan as needed to achieve optimal outcomes.
- Observe and evaluate resident care, providing guidance and support to staff as required to enhance care delivery.
- Collaborate with family members, healthcare providers, and external agencies as necessary to coordinate care and resolve issues.
Qualifications:
- Legal working age and authorized to work in the United States.
- Bachelor’s degree (Required).
- Strong written and verbal communication skills, with the ability to effectively interact with residents, families, and the healthcare team.
- Ability to follow detailed instructions and adhere to organizational standards.
- Excellent interpersonal skills and the ability to handle sensitive matters with tact and professionalism.
- Ability to work independently as well as part of a team, with a focus on providing exceptional care.
- Willingness to work overtime, as needed.
- Current and active RN license (Preferred).
- Previous experience in utilization review or a related field (1 year preferred).
Work Environment & Schedule:
- Full-time, Monday to Friday
- Day shift
- In-person position at our Bensenville, IL location (Relocation required prior to starting).
