Job Summary
The Care Manager plays a crucial role in providing support and guidance to individuals facing various challenges. Care Management is the coordination of all services (behavioral health, medical and social services, and more) for the enrolled adult or child. By coordinating services, it is easier for clients with chronic conditions and/or behavioral health issues to receive the right preventative services, at the right time and in the right amount.
Duties
- Activities include but are not limited to the following:
- Conducts assessments, evaluates needs, establishes and maintains care plan and maintains referrals for enrollees.
- Assures supports are in place including family and peer contacts and all initial associations are established and maintained.
- Develops Plan of Care with client and service providers.
- Collaborates with all services providers and establishes service team communication plan.
- Monitors goals on a continuing basis and ensures service team is communicating.
- Monitors that care plan is relevant to health home policies and procedures.
- Consults with family members and social supports to maintain support consistency.
- Conducts face to face visits in community, institutions, homes, etc.
- Advocates for additional services and associations as appropriate.
- Adhere to mandatory abuse reporting laws and HIPPA requirements.
- Maintains current care management documentation and information regarding care management activities within the required health IT system.
- Completes mandatory trainings on yearly basis or as directed.
Experience
- Bachelor’s degree or Masters in social work, psychology, sociology, human services or related field.
- 1-2 years’ experience in Human Services, primarily Mental Health, Substance Abuse and medical information.
- Basic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel).
- This position has regular and substantial unsupervised or unrestricted physical contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under OMH (Office of Mental Health).
- Travel is required. Must have a valid NYS Driver’s License.
- Must possess the ability to make independent decisions when circumstances are warranted.
