Job Summary
Supervises nursing staff and medical screener. Performs nursing assessments and crisis medical clearance. Fills physician orders, administer medications, takes and monitors vital signs, coordinates medical care, and assesses signs and symptoms of substance intoxication and withdrawal. Performs discharge planning and care coordination.
Adheres to all applicable state and federal regulations; ensures compliance with the organization’s policies and procedures, code of conduct, and all applicable regulatory requirements.
Provides and documents regularly scheduled supervisions of nursing staff.
Participates in interviewing and hiring of nursing staff and provides training and orientation to new hires.
Participates in all required staff meetings, training courses, and needed supervision.
Provides oversight to medical screeners in the coordination of psychiatric schedules, prioritizing those clients with immediate needs.
Collaborates with outpatient providers and hospital staff to obtain medical records for upcoming medical appointments.
Reconciles medication upon patient discharge from hospital or facility.
Other Duties
Performs additional duties, as directed by management.
Serves as liaison between client and provider when medications refills are needed.
Provides basic and emergency medical care and makes proper referrals.
Transports clients, as needed.
Facilitates group activities related to client health.
Triages psychiatric consult requests for medical providers, newly discharged clients (i.e., hospital inpatient and other outside sources in the community).
Provides education and training on health related and medication issues to clients and families.
Intervenes effectively in a crisis, including supporting staff working with a client in crisis, providing nursing assessment, and communicating findings with a clinician.
Qualifications, Experience, and Education
Currently licensed as a Registered Nurse (RN) in the Commonwealth of Massachusetts or Massachusetts waiver obtained for Licensed Practical Nurse (LPN).
Healthcare Provider Basic Life Support (BLS) level of Cardiopulmonary Resuscitation (CPR) or the ability to obtain within the first 5 months of employment.
Minimum two years of supervisory experience preferred.
Minimum 2 years of experience in case coordination preferred including ability to :
Assess clients and advocate for needed treatment.
Collaborate with local physicians and their staff.
Implement medical recommendations.
Basic knowledge of the Diagnostic and Statistical Manual of Mental Illness (DSM V).
Excellent communication skills.
Ability to work with a diverse population with a range of psychiatric disabilities.
Basic knowledge and experience in crisis intervention.
Basic knowledge in the use of psychotropic medications.
Basic knowledge of community resources.
Valid driver’s license and use of personally insured vehicle for agency business.
