What are the responsibilities and job description for the Peer Mentor Intensive HomeBased Therapeutic Care position at Brien Center for Mental Health?
GENERAL DESCRIPTION :
ACTIVELY SUPPORT AND ASSIST YOUTH IN CARE TO EXPERIENCE SUCCESS AND SATISFACTION IN THEIR LIVES BY IMPROVING THEIR QUALITY OF LIFE BY MODELING, BEING AN ACTIVE LISTENER, AND SHARING THE REALITY OF WELLNESS AND RECOVERY. THE PEER MENTOR WILL PROMOTE PERSON CENTERED SELF DETERMINATION AND DECISION MAKING THROUGH THE USE OF THEIR OWN RECOVERY STORIES AND LIVED EXPERIENCE.
ESSENTIAL JOB FUNCTIONS
- Support youth to identify and develop goals
- Model for hope and self determination
- Assist peers with goal setting, problem solving, skill building
- Assist youth to connect to supports i.e. community based services, activities, natural supports
- Communicate a sense of hopefulness and respect
- Provide encouragement and support to youth
- Participate in treatment planning meeting
- Develop and maintain relationships with youth, families, community providers and resources and program staff and other links to the community
- Attend Peer trainings and groups that support recovery and wellness services within the agency and through other recovery and wellness trainings by SAMHSA, RLC, MPower etc.
- Promote recovery based model services to youth through role modeling of successful recovery strategies and skills sets while promoting independence and self advocacy for youth
- Provide transportation to youth when necessary to facilitate treatment plan
ADMINISTRATIVE FUNCTIONS
Ability to complete required note keeping, regarding the development and progress of the group, activity or workshop
Represent the agency in a professional manner at all times
Complete all required paperwork in a timely manner
Participate in all relevant staff meetings, supervisions and required trainings
Report any incident(s) regarding accidents, injuries, and unusual events to the Program Supervisor and / or Program Director
Follow all agency policies and procedures
Maintain ethical boundaries
Flexibly respond to program scheduling needs and assist other programs when assigned
