ARRT certification required
Primary Source Verification of professional license and / or certification required within 30 days of start date

Documentation of TB skin test screening required
Health Statement required within 365 days of start date
MGB Criminal Background Check required within 45 days prior to start date
MGB Massachusetts CORI required within 30 days prior to start date
MGB Resume required for submission
MGB Skills Checklist required for submission
MGB Confirmation of Assignment / POI Form required upon acceptance of assignment
Additional Details
Submit a color photo taken in past 6 months. No selfies, no filters. Plain background. Symmetrical headshot, shoulders up, look straight / no tilted head. No eyeglasses.
Vaccine required during flu season Sept 15th - April 30th. Document must be uploaded as PDF. No exemptions or waivers (religious / medical) will be considered.
