Job Purpose
The SAFE Case Manager will provide youth case management for youth at risk of or experiencing homelessness between the ages of 12 and 26. Client case management includes housing search, support, and stabilization into the SAFE Program at Bright Futures for Youth. This position works closely with the SAFE Program Manager to utilize workflows, policies and procedures, ensure continuous quality improvement, and coordinate with staff and volunteers. This is a full-time, non-exempt position supervised by the SAFE Program Manager.
Primary Duties and Responsibilities
Case Management
- Provide case management services to runaway, housing insecure, and couch-surfing youth
- Maintain a caseload of up to 25 clients
- Introduce eligible clients to BFFY resources or other community resources as needed
- Work with youth to identify goals and tasks related to employment, education, and housing
- Provide transportation for case plan related support as needed
- Assist with Medi-Cal, CalFresh, CalWorks eligibility and enrollment as appropriate
- Make referrals to Behavioral Health and other mental health service providers
- Help with college and/or career and technical education applications and FAFSA
- Assist with crisis intervention
- Enter client data into BFFY SalesForce database and HMIS systems. Must maintain SalesForce and HMIS updated with all client information and data
- Provide mental health/social emotional learning support in SAFE, NEO, and TFC activities as requested by directors
- Assist with facilitation of group activities including Independent Living Skills workshops, Youth Action Board Meetings, and other support groups as requested
- Participate in collaborative case management meetings and activities
- Attend and participate in all relevant meetings and trainings
- Attend weekly 1:1 supervision with the SAFE Program Manager
Housing Coordination and Post Housing Placement Case Management
- Research housing opportunities and stay on top of local available housing
- Assist with rental applications as needed
- Coordinate with Regional Housing Authority on Housing Choice Voucher wait list openings and applications
- Follow-up with youth placed in safe housing alternatives and provide tenant/landlord support as needed
- Complete inspections and assist with needs of tenants living in BFFY master leased units
- Provide housing supports for stabilization, such as rental assistance, security deposits, utilities assistance, housing supplies/furniture needs, etc.
- Participate in weekly TAY Case Conferencing Meetings and follow up with County and partner agency staff as needed.
Drop In Center Operations and Supervision
· Welcome clients to the center
· Provide basic needs assistance
· Do brief intake/screening for new clients
· Supervise clients that are utilizing the drop-in space
· Ensure drop-in and SAFE pantry are stocked with basic needs supplies as needed
Other Duties and Responsibilities
- Attend program events, activities, life skills/expanded learning activities as needed or requested
- Attend out of county conferences as requested
- Attend and help with fundraising activities as needed or requested
- Attend agency and other program meetings as requested
- Complete professional development, skill-building trainings, and other required trainings as directed by the SAFE Program Manager, Office Manager, Associate Director, or Executive Director
- Provide general office or program support when needed.
- Willing to work occasional weekends and evenings.
- Maintain a professional appearance.
- Always maintain full confidentiality about youth and families.
- Perform other related duties as assigned by SAFE Program Manager, Associate Director, or Executive Director.
Qualifications
Education/Experience
- Case Manager I
- Degree in social work, social services, social and behavioral sciences, sociology, psychology, or related required and 1-year relevant experience required. Or Bachelor’s degree with minimal experience
- Case Manager II
- Bachelor’s degree in social work, social services, social and behavioral sciences, sociology, psychology, or related required and 1 year relevant experience
- Case Manager III
- Bachelor’s degree in social work, social services, social and behavioral sciences, sociology, psychology, or related required and 3 years relevant experience
- Work experience in social services and crisis intervention preferred
- Experience working with youth with trauma, adverse childhood experiences, and homelessness preferred
- Experience working within systems of care preferred
- Lived experience a plus
- Must pass background check
Special Requirements
· Valid California Driver’s License
· Reliable transportation and proof of insurance
· Successful background check
Compensation
- Pay range Case Manager I $20-$23/hour, DOE
- Pay range Case Manager II $24-$27/hour, DOE
- Pay range Case Manager III $28-$30/hour, DOE

- Mileage reimbursement at Federal reimbursement rate
