in our communities, poor economic opportunities, inadequate mental health services,
affordable housing and education. These efforts are designed to strengthen local families

Position Overview
The Health and Wellness Director at Bright Star Community Outreach leads initiatives to
promote health and wellness in the community by addressing the social determinants of
health (SDOH). This role involves creating and managing programs, fostering partnerships,
and ensuring equitable access to resources and services that improve overall community
well-being. The Director will oversee efforts related to access to healthcare, advocacy, health
education, preventive care, and research while integrating evidence-based practices and
community-driven approaches.

Scope of Responsibilities
Program Development and Implementation:
Design and implement programs that address community health and wellness
needs, focusing on areas such as chronic disease prevention, nutrition and
fitness.
Ensure programs are culturally appropriate and responsive to the needs of
diverse populations.
Incorporate evidence-based strategies into program development and delivery.
Community Engagement and Collaboration:
Build and maintain partnerships with local organizations, healthcare providers,
government agencies, and community stakeholders.
Represent the organization at community events, coalitions, and forums to
promote health equity initiatives.
Engage with community members to assess needs and barriers to wellness.
Public Health and Data-Driven Strategies:
Conduct community health assessments to identify needs and prioritize
interventions.
Use data and research to guide program design and measure impact.
Assist with grant and resource development to support health initiatives.
Education and Outreach:
Develop and implement outreach strategies to promote health awareness and
access to resources.
Create educational materials and campaigns for various audiences.
Conduct presentations and workshops on health and wellness topics for
community members and stakeholders.
Program Evaluation and Reporting:
Develop metrics to evaluate program effectiveness and community impact.
Monitor progress and provide regular reports to leadership.
Use feedback and evaluation data to improve program quality and outcomes.

Leadership and Advocacy:
Advocate for policies and practices that address health disparities and promote
wellness.
Coach and support staff, volunteers, and community members involved in health
programs.
Serve as a resource and leader in advancing health equity and community
empowerment.
Preferred Background and Qualifications
Bachelor’s degree in public health, social work, community development, or a related
field. Masters preferred.
Minimum of 2 years of experience in public health, community engagement, or
program management.
Knowledge of social determinants of health and community health best practices.
Strong project management and organizational skills, with the ability to manage
multiple initiatives.
Takes pride and pleasure in developing talent and creating engaging workplace
cultures and environments for staff at all levels and from varied backgrounds;
exceptional interpersonal and relationship-building skills.
Basic proficiency in data analysis and program evaluation.
Commitment to health equity, social justice, and community well-being.
