Position Overview
As a Case Manager / Field support Specialist, you will leverage your practical in-home care expertise to provide direct support to our caregivers. You will act as a bridge between clinical care and operational support, ensuring that quality care delivery is maintained at every level. Your role will involve on-site training, quality assurance, real-time problem-solving, and effective communication with both clients and caregivers.
Key Responsibilities
Caregiver Training and Support :
Conduct on-site training sessions for new and existing caregivers, covering essential topics such as personal care assistance, medication administration, fall prevention, client communication, and proper documentation procedures.
- Provide ongoing support to caregivers in the field, troubleshooting issues as they arise.
Quality Assurance :
Regularly visit clients’ homes to observe caregiver-client interactions and assess the quality of care provided.
Problem-Solving :
Address caregiver challenges in real time, offering practical solutions to complex care situations, including client behavior management and medical complications.
Client and Caregiver Communication :
Facilitate clear and effective communication between clients, caregivers, and the care coordination team.
Incident Reporting and Investigation :
Establish and enforce clear procedures for reporting suspected compliance or care-related incidents.
Case Management Support :
Collaborate with care coordinators to review and update client care plans, ensuring that caregivers have all necessary information for delivering optimal care.
Required Skills and Experience
Proven experience as a Certified Nursing Assistant (CNA), Home Health Aide, or similar role, with a strong track record in personal care tasks, medication administration, and client safety.
Basic understanding of medical terminology, disease processes, and common health conditions relevant to home care clients.
Ability to analyze situations, identify potential issues, and make informed decisions to address client needs effectively.
Excellent verbal and written communication skills to interact professionally with clients, caregivers, and healthcare professionals.
A commitment to exceptional customer service, ensuring that both client and caregiver concerns are addressed promptly and with empathy.
Valid driver’s license and reliable transportation.
