Position Overview
We are seeking a qualified and passionate Assessor to assist with essential therapeutic services to residents in skilled nursing facilities, long-term care facilities, and related settings. This role offers the opportunity to make a significant impact on the well-being of individuals who often face complex emotional, psychological, and cognitive challenges.
Key Responsibilities
- Screening: Administer the questionnaires to residents in nursing homes
- Documentation: Maintain accurate records of screenings and follow-up actions, ensuring compliance with facility and regulatory standards
- Collaboration: Work with nursing staff, social workers, and healthcare providers to share results
- Quality Assurance: Ensure screenings are done timely and according to best practices, with an emphasis on resident privacy and dignity
- Compliance: Follow all healthcare regulations, including HIPAA, and facility policies regarding mental health assessments and patient care
Required Qualifications
- Education: Healthcare or mental health professional preferred
- Skills: Ability to assess residents empathetically while maintaining professional boundaries
- Strong communication skills to discuss sensitive mental health issues with residents, families, and healthcare teams
- Detail-oriented with excellent organizational skills to maintain accurate documentation and records
Physical Requirements
- Ability to communicate effectively with residents, families, and healthcare teams
- Ability to travel within the facility as needed for assessments
