DON Position Summary:
The Director of Nursing will oversee all aspects of the nursing department. This includes all staffing needs, resident physical and psychological needs, and enforcement of the facility policy and procedure manuals. The person in this position will report directly to the Administrator.
Responsibilities:
- Supervision of nursing staff. This includes:
- Checking MARs weekly to make sure all signatures and proper documentation are present.
- Ensuring proper medication administration by Registered Medication Aide.
- Reviewing monthly nursing staff schedule.
- Arranging for coverage of nursing staff in case of no call no show, staff calling in sick, or vacation requests with little to no overtime.
- Review of task sheets to ensure proper documentation and implementation of Resident Service Plan.
- Enforce policy and procedures.
- Ensure accuracy of narcotics count.
- Ensure proper transcription of new orders to MARs.
- Tracking resident monthly ancillary charges and submitting to Business Office Manager by the 23rd of each month (make sure Resident Care Coordinator is doing also).
- Ordering nursing supplies as needed.
- Preparation and implementation or Resident Service Plans on each resident within 72 hours of admission, with the goal of completion being done at admission.
- Review and update Resident Service Plans on each resident at 30 days, annually or as needed depending the resident care level and unit residing.
- Completing employee evaluations, including review of evaluation with employee within the evaluation date payroll period of anticipated employee raise.
- Arranging and/or conducting monthly in-service training for nursing and all other departments staff.
- Write monthly nursing schedule and adapt as necessary.
- Conducting monthly staff meetings to address issues/concerns.
- Completion of tuberculin testing upon hire for new employees.
- Conducting assessments using the Uniform Assessment Instrument when the prospective resident is a higher level of care.
- Ensure appropriate transfer of MARs from month-to-month.
- Arranging resident bath and laundry schedule.
- Hiring of nursing staff and ensuring the facility has all the appropriate paperwork.
- Assessing residents in emergency and non-emergency situations.
- Appropriate administration of standard and PRN medication orders.
- Addressing family and resident concerns. Proper reporting of these concerns to the Administrator.
- Administering wound care and injections or use Home Health Services.
- On call 24 hours per day for staffing needs, staff questions and resident emergencies.
- Working shifts to cover for call-ins 24 hours a day. Employee will be given a day off to compensate for time work when possible.
- Taking telephone orders from physicians and reporting appropriate resident information to physicians.
- Ensure facility emergency kits have appropriate supplies at all times. Monthly checks of expiration dates and document on log.
- Complete all other tasks as requested.
- Conduct chart reviews monthly.
- Complete on-sight/oversight forms monthly.
- Comply with all regulations mandated by the Virginia Department of Social Services, Licensing Division.
- Conducting initial staff disciplinary process.
- Conduct facility operations as instructed in the absence of the Administrator and Director of Marketing.
- Inform Administrator of resident issues on a daily basis.
- Exhibit teamwork with all departments.
Wound Care
1. Administering wound care per physician’s order.
2. Documentation on MAR and in resident nursing notes each time wound care is provided and any changes in the wound and/or resident condition.
3. Contact physicians and obtain physician orders regarding wound care treatment and order wound care supplies. Document in nursing notes. Supervise the completion of wound care requirements per physician orders that may be conducted by direct care staff.
Assessments and Reassessments
1. Completion of all initial assessment documents prior to admission including:
a. Private Pay Uniform Assessment Instrument (UAI).
b. Initial Assessment Packet
2. Completion of all assessment documents required for re-admission including:
a. Private Pay Uniform Assessment Instrument when applicable.
b. Updates to Initial Assessment Packet
Qualifications:
Must possess a valid license as a Licensed Practical Nurse or Registered Nurse. Formal training in gerontology/geriatrics preferred. Experience working with older adults is preferred. Must have an outgoing personality, patience and compassion. Flexibility and the ability to communicate in a pleasant voice and manner at all times are required.
Supervisory Responsibility:
This position requires direct supervision of nursing staff.
Equipment to be used:
Must have basic computer skills. Must be able to use all appropriate nursing and/or facility equipment.
Mental and Physical Demands:
The Director of Nursing must be able to meet a work schedule with multi-faceted tasks. Physically, he or she must have the stamina to carry out repetitive activities and the hand dexterity to perform nursing duties.
He or she must have the physical strength to lift and manipulate weights of at least 40-pounds.
Working Conditions:
This position requires that the Director of Nursing works throughout the building.
Dress Code:
The person in this position must always dress professionally and wear the company name badge. Appropriate clothing is professional dress or nursing attire.
Employment is contingent upon proof of U.S. Citizenship or work permit, a valid Virginia driver’s license, a valid Commonwealth of Virginia nursing license, criminal background check and passage of a drug screening.
