Job Summary :
Join our compassionate and collaborative team as a Community Health Worker and make a difference in the lives of individuals and families in our community. We are looking for someone who is passionate about improving community health, has excellent communication skills, and is dedicated to helping others. As a Community Health Worker, you will play a vital role in connecting health providers with community resources, educating patients on reducing health risk factors, and empowering individuals to take control of their health.
In this role, you will facilitate communication between patients and healthcare providers, troubleshoot barriers to care, engage with the community through outreach, and assist in developing care management processes. You will also build community capacity by increasing health knowledge and self-sufficiency, collaborate with care teams to overcome healthcare barriers, and connect patients with necessary resources.
