Job Summary: The Residential Program Manager serves as an interdisciplinary team member for each person under their direct oversight. The Manager will coordinate with internal and external agencies to ensure care and support for individuals in services. The Program Manager will ensure compliance with State Regulations.
Duties and Responsibilities required of the position:
1) Coordinating and completing assessments.
2) Completing annual assessments and Lifetime Medical Histories along with The Health Risk Screening Tool for all assigned individuals.
3) Participating in the development of the ISP, ISP annual update and ISP revision.
4) Attending all ISP meetings.
5) Reviewing the ISP, completing annual updates, and revisions.
6) Reporting content discrepancy to the SC, as applicable, and plan team members.
7) Implementing the ISP as written. Develop Behavioral Support Plans/Restrictive Behavioral Support Plans.
8) Supervising, monitoring, and evaluating services provided to the individual.
9) Reviewing, signing, and dating the monthly documentation of an individual’s participation and progress toward outcomes.
10) Reporting a change related to the individual’s needs to the SC, as applicable, and plan team members.
11) Reviewing the ISP with the individual
12) Documenting the review of the ISP
13) Providing the documentation of the ISP review to the SC, as applicable, and plan team members as required
14) Informing plan team members of the option to decline the ISP review documentation as required
15) Recommending a revision to a service or outcome in the ISP as provided
16) Coordinating the services provided to an individual.
17) Coordinating the training of direct support professionals in the content of health and safety needs relevant to each individual.
18) Developing and implementing provider services as required
19) Compliance with Waiver, Incident/Risk Management, Quality Management plans of the company.
20) Completion of weekly billing to Promise.
21) Supervise the assigned programs, program supervisor
22) Ensure compliance from a regulatory standpoint.
23) Serve as the point person for incidents.
24) Orient new staff, complete hiring & interviewing.
Job Title of Immediate Supervisor: Director of Residential Services
This position does require travel between assigned programs to monitor compliance, wellness checks and supervisory visits. Hours are typically M-F 8am-4pm, with on-call responsibilities.
Minimum Knowledge, skills, and abilities, experience or professional qualifications required :
1) A master’s degree or above from an accredited college or university and 1-year work experience working directly with persons with Intellectual Disabilities.
2) A bachelor’s degree from an accredited college or university and 2 years work experience working directly with persons with Intellectual Disabilities.
