Unit Based Services Program Assistant
Location: Twin Cities and surrounding areas - Minnesota
Employment Type: Full-Time
Wage: $22-27 hourly

Key Responsibilities (Current):
- Provide direct care and support to individuals living independently, helping them acquire essential adaptive skills.
- Assist individuals in developing personalized 24-Hour Emergency Back-Up Plans.
- Support the Designated Coordinator with clerical tasks, ensuring efficient service delivery.
- Document progress, report incidents, and maintain compliance with 245D regulatory guidelines.
- Collaborate with a wide range of professionals, including waivered service providers, mental health teams, and social workers.
- Participate in an on-call rotation for 24-Hour Emergency Assistance services.

What We’re Looking For:
- Compassionate, empathetic, and patient, with strong problem-solving skills.
- Strong organizational and time-management skills.
- Valid driver’s license and access to a reliable, insured vehicle (mileage reimbursement provided)
- Must pass criminal/registry background checks in accordance with Minnesota DHS guidelines.
- Hold at minimum a 2-year degree in Human Services or a related field, AND at minimum 2 years of direct experience working with individuals with Intellectual/Developmental Disabilities or related conditions
Working Conditions:
The Unit Based Services Program Assistant works in various community settings, frequently traveling to clients' homes. Each day brings new challenges and opportunities to support individuals in their unique environments. Light physical assistance may be required. Reliable transportation is essential.
