About the Job
We are seeking a highly skilled and compassionate Hospice Social Worker to join our team. As a key member of our interdisciplinary team, you will play a critical role in providing medical social work services and coordinating with healthcare professionals to ensure optimal patient outcomes.
The successful candidate will have a graduate degree in social work (MSW) from a Council on Social Work Education-accredited program, with a minimum of one year's experience in healthcare, preferably in hospice care. You will be expected to maintain current licensure according to Medicare guidelines and possess excellent verbal and written communication skills.
