- Conduct appropriate evaluations, assess home environment and identify equipment needs relative to speech/language function
- Identify functional speech defects and establish a plan of care to improve patient's function
- Administer speech therapy program utilizing specialized therapeutic technique and/or equipment and recommend communication devices/aids as indicated
- Maintain accurate and timely documentation
- Participate in case conference as required
- Ensure compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
- Master's degree in Speech Language Pathology with current licensure to practice in state of operation
- 1 year experience in Speech Language Pathology
- Home health experience preferred
- Commitment to clinical and documentation excellence
