We are seeking compassionate and reliable Home Health Aides (Private Duty) to provide personalized care to individuals in the comfort of their homes. As a Home Health Aide, you will play a crucial role in assisting clients with daily living activities, ensuring their well-being, and providing companionship. Open Assignments :
- Lake Sheffield Ohio - Monday-Thursday 8 AM to 12 PM
- Middleburgh Heights - Wed-Thursday and Friday 1 PM to 6 PM
- Richmond Heights - 7 days a week 9 AM to 1 PM

Job Responsibilities
Provide assistance with daily tasks as directed in the client's personalized plan of care, including but not limited to : feeding, meal preparation, linen changes, light housekeeping, providing transportation to doctor appointments, outdoor activities, and shoppingProvide companionship, personal and hygiene care as directed in the client's personalized plan of care, including but not limited to : bathing, grooming, oral care, dressing, undressing, toileting activities and assisting with measurements, if necessaryAssist clients with active and passive mobility work defined by the plan of care, and documenting observed changes to the Director of NursingAdhere to the policies, procedures, and protocols of the agency of employmentAdhere to HIPAA laws and ensure confidentiality and security of the client's medical informationJob Requirements
High school diploma or GEDMinimum one (1) year of documented home care assistance and / or direct care experience at an assisted living or skilled nursing facility, etc., is preferredPreferred : Successful completion of a nursing assistant or certified home health aide programCertified CPR and negative TB skin test or chest x-rayMust have valid drivers license and auto insurance (proof of insurance is required)Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.
