BrightStar Care is looking for a dedicated and compassionate RN Case Manager to provide in-home infusion and injection services to patients in the St. Paul area. This flexible position requires availability 3 days a week between 8:00 AM – 5:00 PM, with travel within a 30-mile radius. Mileage is reimbursed. Job Responsibilities:
- Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
- Collaborate with multidisciplinary team members, partnering pharmacies and providers to optimize patient care and outcomes.
- Educate patients and their families on health maintenance and disease prevention.
- Ensure accurate documentation and compliance with all regulatory guidelines.
Requirements:
- Active RN license in the state of practice.
- Previous experience in a home health or acute care setting is advantageous.
- Exceptional interpersonal and communication skills.
