**Available Shifts: **
- 24/7 clients
- PRN shifts
- Infusion clients
**What We’re Looking For: **
- An unrestricted LPN or RN license in Wisconsin.
- Current CPR certification and a negative TB skin test or chest x-ray.
- Ideally, 1 year of experience in a home health agency, primary care clinic, or health facility.
- Valid driver’s license, car insurance, and a reliable vehicle.
**Your Role: **
- Deliver personalized, compassionate care tailored to each client’s Plan of Care.
- Keep an open line of communication with clients’ families about care plans and health updates.
- Monitor and share any changes in client conditions with the Director of Nursing and the appropriate team members.
