The Registered Nurse is responsible for providing home health clinical services. The registered nurse is responsible for quality, adequacy of services provided and may, where required, supervise staff. The RN Field Specialist is responsible for ensuring that care is delivered appropriately and the standards of quality are adhered to.

Responsibilities :
Participates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care
Patient Assessment : Perform comprehensive assessments of patients' health status, including physical, emotional, and social needs.
Care Planning : Develop and implement individualized care plans based on assessment findings, patient needs, and best practices.
Patient Education : Educate patients and their families on managing illnesses, medications, and overall health maintenance.
Coordination of Care : Collaborate with multidisciplinary teams, including physicians, social workers, and therapists, to ensure comprehensive patient care.
Health Monitoring : Regularly monitor patients' conditions, documenting changes and reporting them to the appropriate healthcare team members.
Documentation : Maintain accurate and timely documentation in compliance with legal, regulatory, and organizational standards.
Emergency Response : Provide immediate care and interventions in emergency situations and assess when to escalate care.
Requirements :
Current unrestricted RN license in the state of NJ
Comfortable with school trips / 1 : 1 with students
Comfortable with home visits, reassessments
Comfortable with all ages from pediatric to geriatric patients.
Wound care experience
Competent with blood draws, IV infusion therapy (peripheral and central)
Must be available at least 30 hours on a weekly basis.
Able to assist with skilled staffing needs.
Preferred : Bachelor's degree in Nursing. Must have graduated from a National League of Nursing accredited program
Three (3) to five (5) years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred.
Understands regulatory compliance with state and federal regulations
Adhere to HIPAA and maintain client confidentiality
