Responsibilities
- Make a difference for your clients and their families in their own home!
- Implementation of all physical therapy modalities to patients
- All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization
- Improves or minimizes residual physical disabilities of the patient
- Returns the individual to optimum and productive level within patient’s capabilities
- Participates with all other home care personnel in patient care planning
- Reviews initial history and reviews initial evaluation
- Consults with physicians regarding patient’s progress
- Prepares clinical and progress notes
- Helps communicate the plan of care
- Consults with family and agency personnel
- Ability to do extensive bending, lifting and standing on a regular basis
Qualifications
- A graduate of a PTA Program recognized by the American Physical Therapy Association or other National accepted Council or Association
- Active license (PTA)
- Must pass criminal background check
- Must have current CPR certification or willing to complete
- Must have active driver's license and clear driving record
- Computer skills
- Ability to exercise initiative and independent judgment
