Job description Full-Time Home Health RN Case Manager for Private Duty Services (PDS) $75,000.00 DOE Job Summary BrightStar Care - Home Health Care Agency is hiring a full-time field RN Case Manager for our San Antonio agency. We are seeking highly motivated and professional applicants with prior home health experience, case management experience, and a compassionate sense of urgency. What You'll Do : Provide RN case management services for PAS and PDN departments under the direction of the Director of Nursing (DON).Determining eligibility and initiating a comprehensive plan of care (POC) based on the needs of each patient and maintaining optimal communications with clients, clinicians and departments. Evaluating patients for eligibility for Home Health PAS and / or PDN services. Conduct field Starts of Care and field supervisory visits to ensure safe and Developing a comprehensive Home Health plan of care incorporating patient goals and expected outcomes of treatment. Thorough, concise and timely documentation via EMR and communications via company approved platforms. All other appropriate duties as assigned .What We'll Love About You : Strong electronic documentation and follow-up skills required. Computer literacy and proficiency with Microsoft Office suite and EMR system Strong working knowledge case management Willing to be on an On-Call rotation Willing to travel to see clients as needed Minimum of 1 year supervisory experience as Case Management experience in home health setting, preferably in Private Duty Services.
