As an LPN Supervisor, you will oversee the delivery of care in the home health setting, ensuring compliance with health guidelines, and maintaining high client satisfaction. This position requires strong organizational skills, attention to detail, and the ability to communicate effectively with the interdisciplinary team.
Key Responsibilities:
- Schedule Supervisory Home Visits: Coordinate and schedule client supervisory visits to ensure that clients are receiving the appropriate level of care.
- Complete Supervisory Home Visits: Conduct home visits to assess the quality of care being provided, ensuring that it meets the client’s needs and complies with regulatory standards.
- Compliance Assurance: Monitor and ensure that all home health care practices meet local, state, and federal regulatory guidelines, ensuring a high standard of care and client safety.
- Communication with Disciplinary Team: Communicate the outcomes of supervisory visits with the interdisciplinary team to ensure client satisfaction, address concerns, and ensure continuity of care.
- Prepare Compliance Reports: Document and prepare regular compliance reports, highlighting visit outcomes, corrective actions (if any), and other necessary follow-up.
- Other Duties as Assigned: Assist with various tasks as needed to support the operational needs of the agency.
Qualifications:
- LPN License in the state of [State Name].
- Reliable Transportation: Ability to travel to clients' homes for supervisory visits.
- Prior Home Health Experience: Experience in home health care is preferred, though not required.
- Ability to Use a Laptop: Comfortable using a laptop and various software applications to manage schedules, documentation, and reports.
