The Registered Nurse Case Manager will work with our Enterprise Director of Nursing and Branch Manager to oversee the care provided to our clients. A typical day may include performing new client assessments, overseeing the care of existing clients (personal care and skilled), and care coordination with skilled service organizations and hospices. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with clients and their families. The Case Manager will work primarily (approximately 75%) in the field (local travel) and 25% of the time in the office.

Job Responsibilities
- Provide care as defined within the scope of the state’s nurse practice act
- Make the initial evaluation visit and regularly reevaluate the client’s nursing and/or personal assistance needs
- Initiate appropriate preventive and rehabilitative nursing procedures and counsel client and family on nursing related needs
- Administer agency policies and procedures
- Assist with assessing the competency of staff to promote quality, continuity and safety of client care
- Promote home care referrals in the health care community
Required Skills and Qualifications
- Registered nurse with current unencumbered license to practice professional nursing in the state the job is located
- Minimum of three (3) years of documented experience required. Two (2) or more years of clinical experience with one (1) year experience in a home health agency or case management preferred
- Understanding of regulatory compliance local, state and federal laws, Joint Commission, OSHA and HIPAA
- Flexibility and good time management skills A MUST
- Ability to travel locally
