Home Infusion RN
Description of the role:
Are you an experienced RN with a passion for infusion therapy and patient-centered care?
BrightStar of Lake County Indiana is seeking a Home Infusion RN to join our team. The Infusion RN will be responsible for coordinating and performing infusion therapy in the home setting. This role requires excellent clinical and communication skills, as well as a strong attention to detail. The Infusion RN will work closely with physicians and other healthcare professionals to provide the highest quality of care to our patients.
Responsibilities:
- Coordinate and perform skilled infusion nursing care in the home setting
- Provide teaching to patients and caregivers on home medication management and medical conditions
- Collaborate with healthcare professionals to establish priorities and achieve patient care goals
- Perform patient assessments, initiate and monitor infusion therapy and provide psychosocial support
- Develop and revise care plans based on patient needs and physician orders
- Document and submit patient care reports accurately and in a timely manner.
- Monitor lab results and ensure communication with the pharmacy and other healthcare providers
Requirements:
- Active RN license in the state of Indiana
- Minimum of 2 years of experience in infusion therapy (peripheral IV insertion, PICC/midline/central line maintenance, Port-a-cath accessing)
- Strong clinical skills and knowledge of infusion treatments
- Excellent communication and interpersonal skills
- Ability to work well in a fast-paced environment
- Attention to detail and ability to multitask
