Position Summary The Resident Care Coordinator assists the Health Services Director in ensuring residents receive exceptional care and support. This role involves staff supervision, training, and maintaining compliance with company policies and healthcare regulations.
Key Responsibilities

Assist in overseeing resident care services, ensuring compliance with all applicable regulations.

Utilize OnShift to support scheduling and staffing needs for Med Techs and Resident Assistants.

Ensure care plans and service assignments are completed and documented in CareStream.

Assist with staff training, including new hire orientation and ongoing education via CareAcademy.

Conduct routine health assessments, including 30-day post-move-in evaluations and biannual reviews.

Monitor physician orders, medication administration, and treatment plans to ensure proper implementation.

Oversee medication administration records (MAR) to verify accurate documentation and compliance.

Coordinate with home health, hospice, and other healthcare providers for resident care.

Support quality assurance audits, incident reporting, and inter-shift communication.

Communicate effectively with residents, families, and healthcare professionals.
Qualifications

Strong leadership, interpersonal, and communication skills.

Experience in senior living, RCFE, and dementia care preferred.

Ability to meet state or provincial healthcare requirements.

Maintain CPR and first aid certification.

Proficiency in electronic health records, such as Yardi.
