JOB DESCRIPTION
Title: Kinship Care Navigator & Family Support Case Manager
Grade Level: H Case Management
Supervised By: Program Manager
Starting Wage: $21, or $22.26 (Bilingual proficiency add-on) hourly, non-exempt.
Full-Time Equivalency: Full-Time, 40 hours per week
Purpose: The purpose of the Kinship Care navigator/Family Support Team case manager on the Brigid Collins Family Support Team is to provide guidance, support, and resources to relatives and close family friends who are raising children outside of the traditional foster care system, families recovering from substance use disorder, and families who have been through the Skagit County Children’s Advocacy Center. By offering comprehensive information, referrals, and advocacy, the Kinship Navigator aims to empower caregivers with the tools they need to navigate legal, financial, educational, and emotional challenges. Our goal is to strengthen families, promote stability, and enhance the well-being of both caregivers and children by ensuring they have access to the services and support necessary to thrive.
Primary responsibilities include, but are not limited to:
· Responsible for program planning, development, implementation, and outcomes.
· Deliver case management support service to families to maintain family stability, support caregivers to be able to care for children, and prevent further trauma.
· Provide active outreach to engage clients throughout Skagit and Island County.
· Educate other service professionals regarding Kinship Care, Mothers in Recovery, and the Skagit County Children’s Advocacy Center.
· Coordinate with other service professionals within the community.
· Attend and/or facilitate staffing/multi-disciplinary team meetings to establish and maintain collaborative and coordinated service plans.
· Develop and monitor the client’s/family’s service plan.
· Provide needed referrals and linkages to families.
· Support peer-to-peer interactions by helping to establish a network of kinship services.
· Establish and maintain the required caseload.
· Maintain documentation and reporting procedures.
· Learn and maintain model fidelity to the State of Washington’s Kinship Navigator Program.
· Learn and maintain required training in crime victim’s advocacy for children.
- Meet and maintain standards for all evidence-based practices, including quality assurance, improvement, and training activities.
- Fulfill various contract expectations assigned.
- Other related duties as assigned.
