Hours : 40 hours per week | 8a – 4 : 30pm
Location | Department : Day Center & In-Home | Thome PACE
Overview :
The Certified Nurse Aide (CNA) performs patient care activities and other tasks to assist the primary care, therapy, and adult day health team members.
Thome PACE® serves Jackson, Lenawee, and Hillsdale counties providing participants with a wide range of medical and supportive services in our Day Center located in Jackson, Michigan or in the comfort of their own homes.
What you'll do :
- Assists in providing and / or maintaining optimal physical and emotional comforts to participants.
- Assists participants toward achieving maximum self-reliance and independence.
- Communicates with participants, families, staff, and the staff of other agencies.
- Participates in interdisciplinary team meetings.
- Provides basic ADL nursing care.
- Documents treatments / procedures, observations, nursing care, and participant activities.
- Reports changes in the participant’s condition to the pertinent interdisciplinary team member.
- Demonstrates active participation in QI processes.
- Participates in the process of teaching and learning with others.
