What You’ll Do in This Role:
The Concierge Provides non-clinical home care support to participants, following interdisciplinary team guidance. Services are 1:1 and delivered in participants' homes or contracted settings (Adult Foster Care, Assisted Living, or Skilled Nursing Facility).
Primary Responsibilities:
- Provide 1:1 supervision and companionship to participants.
- Facilitate activities and offer gentle redirection as needed.
- Perform light housekeeping (e.g., washing dishes, sweeping, tidying up).
- Prepare nutritious and satisfying meals.
- Accompany participants on bus rides to homes, the PACE Center, or appointments.
- Assist in the Day Center as needed.
✅ What You’ll Need:
- At least one year of experience working with frail elderly or long-term care populations.
- Reliable transportation and a valid Michigan Driver’s License.
- Possess current CPR certification or willing and able to obtain as offered by LifeCircles.
