Position : Home Care Concierge
Hours : 40 hours per week | 8 : 00 AM - 4 : 30 PM

At LifeCircles PACE, we believe in creating a supportive and dignified environment for older adults to thrive. As a Home Care Concierge , you’ll provide essential, non-clinical care and companionship that make a meaningful difference in the lives of our participants.
What You’ll Do :
As a Home Care Concierge, you will :
- Provide 1 : 1 supervision and companionship to assigned participants.
- Facilitate activities and provide gentle redirection when needed.
- Perform light housekeeping tasks such as washing dishes, sweeping, and tidying up.
- Prepare meals to ensure participants enjoy nutritious and satisfying food.
- Accompany participants on bus rides to their homes, the PACE Center, or external appointments.
- Provide additional support in the Day Center as requested.
Your role will primarily take place in participants’ homes or contracted care settings, including Adult Foster Care, Assisted Living, or Skilled Nursing Facilities.
