Hours : 40 hours per week | Monday-Friday | 8am-4 : 30pm

Status : Full time | Exempt

Location | Department : LifeCircles PACE | Holland

Overview :

The Occupational Therapist (OT) will provide comprehensive occupational therapy services to frail, elderly participants in a PACE program. The OT will be responsible for assessing, planning, and implementing individualized treatment plans to promote independence, safety, and quality of life for participants. This role requires a strong knowledge of current occupational therapy practices, long-term care regulations, and the ability to work in a multidisciplinary team environment.

What you’ll do :

  • Conducts initial and ongoing assessments to evaluate the rehabilitative needs of LifeCircles participants. Identifies participant goals and develops, implements, and revises individual care plans as per LifeCircles policy.
  • Assesses the need for adaptive equipment or durable medical devices, and provides recommendations to the IDT for approval and implementation.
  • Plans, organizes, and conducts occupational therapy programs.
  • Selects activities that will help individuals learn work and life-management skills within the limits of their mental or physical capabilities.
  • Recommends changes in residents' work or living environments, consistent with their needs and capabilities.
  • Completes and maintains resident records.
  • Provides training, education, and support to caregivers and family members to ensure effective care and support for participants in their home and community settings.
  • Conducts training programs in occupational therapy techniques and objectives.

What it takes :

  • Must be licensed to practice as an Occupational Therapist in the State of Michigan with an unencumbered, active license.
  • Knowledge of current Occupational Therapy practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Excellent verbal and written communication.
  • Ability to work effectively as part of a multidisciplinary team.
  • Comfortable in a self-directed work environment with the ability to manage time and responsibilities independently.
  • Ability to maintain confidentiality and adhere to all HIPAA guidelines.
  • Ability to move and assist participants, including the use of equipment such as wheelchairs, walkers, and other assistive devices.
  • Ability to work in a variety of settings, including participants' homes and the PACE center.
