Service Coordinator, BSW
Schedule : Exempt, minimum 40 hours / week | Monday - Friday, flexible hours
Location / Department : Affordable Housing | Kentwood & Caledonia, MI
Overview :
Join our team as a Service Coordinator , where you’ll play a vital role in helping residents maintain their independence and well-being. In this position, you will identify residents' needs, provide assistance, and connect 92 residents at our Kentwood and Caledonia apartment communities with supportive services that promote safe, independent living.
What You’ll Do :
- Coordinate supportive services to enhance residents' well-being and independence.
- Develop, organize, and maintain service programs that deliver meaningful benefits to residents, including nursing, podiatry, food services, music therapy, and more.
- Provide comprehensive case management and referral services tailored to individual resident needs.
- Document and submit case notes monthly using AASC software.
- Complete resident assessments and interact with residents, families, and others to evaluate their needs.
- Provide direction and resources to residents and families to support resident independence.
- Oversee volunteers, program aides, and service agencies to ensure high-quality services.
- Submit monthly case notes and annual reports via Service Coordinator Case Management software.
