Location : Must be willing to travel between all Brio locations depending on schedule needs
Position Overview : Works independently with the Medicare and diabetic population to perform Annual Wellness Visits and Diabetic Care Visits to identify gaps in care, safety concerns, and social needs. Prepares charts for scheduled visits. Performs procedures when applicable during those visits.
Responsibilities :
- Takes ownership of ensuring the "Brio Way" is experienced by patients, families, and internal team members. Promotes an authentic, transparent work environment within the clinical areas.
- Helps carry out team short term and long-term goals for Annual Wellness Visit / Diabetic Care Team (AWV / DCT) and ensure efficiencies with processes.
- Prepares charts for scheduled Annual Wellness Visits with patients and ensures patient is up to date on all routine screenings and immunizations.
- Works with Medicare patients to complete Medicare's yearly Health Risk Assessment when the patient comes in for any blood work needed prior to the visit with the provider.
- Will meet directly with each patient for approximately 20 minutes up to 1 hour to review paperwork to ensure patient’s chart is up to date and perform vitals.
- Provides communication with providers regarding patient needs identified in scheduled Annual Wellness and Diabetic Care Visits, such as gaps in care in care, safety concerns and social needs, for their upcoming follow-up visit.
- Ensures all equipment needed for AWV / DCT appointments remains in working order and are serviced when needed.
- Completes scheduled Diabetic Care Team visits to ensure patients are up to date with recommended screenings and immunizations when the patient comes in for any blood work needed prior to the visit with the provider.
- Performs any point of care injections or procedures per protocol that patients need during scheduled visits.
- Helps to identify patients eligible for an AWV / DCT and schedule / alert provider when needed.
Requirements : Required Education :
Physical Demands : Ability to perform moderate physical activity. May require handling / lifting of children / objects (up to 50 pounds) or standing / sitting / walking for more than four (4) hours per day. Prolonged use of telephone may be required.
Environmental / Working Conditions : Ability to work in an environment with exposure to potentially dangerous / infectious materials and situations that require following extensive safety procedures. Hours may be shortened as business needs dictate.
