This role includes remote phone triage and visits in the field as necessary. The On-Call RN ensures quality and safe delivery of home care and hospice nursing services rendered during non-traditional work hours.
On-Call RN
Duties and Responsibilities :
- Respond to on-call messages that will include making recommendations via phone and making visits to the home of the patient about whom the call is received.
- Communicate on-call outcomes to day shift operations
- Death pronouncements for patients dying at home or in a location where there is not a registered nurse to pronounce death.
- Write progress notes for all telephone contact and all in-person visits.
- Contact attending physician or medical director as needed for any changes in patient status or need for change in physician orders.
- Provide appropriate support at time of death and perform bereavement assessment if requested.
- Participate in orientation and in-service training programs for professional staff.
- Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
- Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which assist in determining policies.
- Accept other assignments as appropriate.
Qualifications :
State / Federal Mandated Licensure or Certification Requirements :
Current state of Connecticut RN Licensure
Bristol Hospital Mandated Educational Requirements :
General orientation at time of hire. Fire / Safety / Infection control annually. CPR, Code Blue & Accucheck annually, IV Medication & Blood Administration at time of orientation. Other programs as mandated by Hospital.
Physical Requirements :
Considerable walking, standing, sitting, stooping and bending. Considerable paperwork and video display terminal work requiring extensive eye usage.
Work Environment :
Normal nursing unit environment, including patient furnishings and equipment.

Strong interpersonal, communication, and negotiation skills.
Ability to form positive, collaborative relationships with hospital staff, patients and families.
Ability to effectively negotiate with internal and external providers of patient care services.
Analytical abilities to assist in obtaining solutions to problems.
Able to work independently and prioritize work.
Able to manage multiple priorities.
Basic knowledge of computers and clinical applications.
Ability to problem solve in a proactive, creative manner using sound judgment based on factual information and clinical knowledge.
Communicate in a clear, concise, and logical manner in oral and written presentations.
