A hospital based representative of home care and hospice whose primary role is to provide effective placement of home care and hospice patients located throughout the hospital. In addition, create relationships with professional influencers and their place of business, e.g. doctor offices, discharge planners, social workers, assisted living communities, rehab, etc…
Functions as an expert in home care and hospice capabilities and answers questions from providers, patients and their families to ensure that the home care and/or hospice program is well understood and that successful transitions occur whenever possible so that patients benefit from home health/ hospice while simultaneously reducing acute care burden.
Help facilitate referral and transition from hospital to home care/ hospice. Introduce concept of home health services, provide brief agency overview.
Evaluate inpatient and emergency department patients for potential home care/ hospice eligibility
Evaluate GIP patients for the potential to transition to home hospice care
Coordinate closely with the ER and inpatient social worker and case management teams
Collect medical, financial, and psychosocial data necessary to determine viability of the patient for admission.
Communicate pertinent clinical information to the Intake Coordinator where the final determination regarding patient’s acceptance for admission will be made.
To network with discharge planners, case managers, physicians for the purpose of positively and honestly promoting facilities programs and increasing appropriate referrals from these sources.
Develop formal weekly and monthly marketing plans in conjunction with the Director to identify and prioritize categories and specific targets of professional influencers and community organizations.
Market BHCH service lines for system and non-system partners.
Monitor all current/ new patients while at the hospital
Perform pre-discharge patient and family assessment to determine understanding and acceptance of discharge plan and orders in conjunction with discharge planning staff to ensure a smooth transition home.
