About the Position:
- This role requires collaboration with clinical and other staff to achieve improved patient care and outcomes during medical visits.
Main Responsibilities:
- Perform assessments, diagnoses, treatments, and evaluations/monitorings for patients
- Collaborate with clinical and other staff to improve patient care and outcomes
- Participate in training, team meetings, and huddles to facilitate delivery of patient-centered care
Requirements:
- Graduate of an accredited nurse practitioner program
- Licensed as an advanced practice registered nurse (APRN) in South Carolina with ability to see patients of all ages
- Federal and state prescriptive authority
- Minimum three years nurse practitioner experience
