About the Role
The Element Care Social Worker plays a key role in planning, implementing, and evaluating care plans that meet the objectives, standards, and policies of the PACE model of care.
The successful candidate will demonstrate proficiency in providing traditional social work services in a professional and respectful manner, with the goal of helping older adults live safely and comfortably in their homes and communities for as long as possible.
Requirements
- Current Master's level Social Work licensure in Massachusetts (L.I.C.S.W. or L.C.S.W.)
- At least 1 year of experience in Social Work, providing clinical or case management services to a geriatric population
- Current CPR certification or ability to become certified
- Treat all participants with respect and professionalism
- Strong verbal, written, and listening skills, with the ability to multitask in a fast-paced environment
- Covid vaccine required
