Broad Reach Hospice is seeking a qualified SOCIAL WORKER to join our team of dedicated healthcare professionals.
As a Social Worker at Broad Reach Hospice you will collaborate with our interdisciplinary team to participate in assessments, develop care plans, carry out psychosocial treatments, and manage discharge planning for both in-patient and home care patients.
Schedule: Part-Time - 3 or 4 days per week.
Pay Range: $35 - $55 per hour, depending upon experience.

Social Worker Responsibilities
Perform comprehensive initial psychosocial and bereavement evaluations and assist in the development and implementation of the plan of care.
- Continually assess patient/family needs and counseling.
- Provide short term crisis intervention and individual/family counseling.
- Observe, evaluate, and report on psychosocial, spiritual, physical, and financial condition affecting the patient/family.
- Develop and maintain communication with community agencies/services to promote interagency cooperation and to facilitate referrals.
- Travel extensively to patient homes, hospitals, and nursing facilities throughout Broad Reach Hospice's service area.
- Bachelor's or Master’s degree in Social Work.
- Massachusetts State Licensure at LSW, LCSW or LICSW level.
- Strong background in Social Work,
- Experience in health services delivery.
- Strong communication skills.
- Strong team working skills.
- Experience in conducting assessments of patient/family psychosocial history.
- Strong problem solving and task prioritization skills.
