Broad ReachHospice is seeking a qualified SOCIAL WORKER to join our team of dedicatedhealthcare professionals.
As a Social Worker at Broad Reach Hospice you willcollaborate with our interdisciplinary team to participate in assessments,develop care plans, carry out psychosocial treatments, and manage dischargeplanning for both in-patient and home care patients.
Social Worker Responsibilities :
- Perform comprehensive initial psychosocial andbereavement evaluations and assist in the development and implementation of theplan of care.
- Continually assess patient / family needs andcounseling.
- Provide short term crisis intervention andindividual / family counseling.
- Observe, evaluate, and report on psychosocial,spiritual, physical, and financial condition affecting the patient / family.
- Develop and maintain communication withcommunity agencies / services to promote interagency cooperation and tofacilitate referrals.
- Travel extensively to patient homes, hospitals,and nursing facilities throughout Broad Reach Hospice's service area.
