Job Summary : The Licensed Practical Nurse (LPN) is responsible for providing directnursing care, monitoring the health care of individuals, and assistingindividuals with accessing appropriate health care within the LPN scope ofpractice. The LPN is also responsible for monitoring the medication management.Clinical assistance is under the direction of the RN Nurse Manager.

Brockton Home HealthCare is quickly expanding and we are hiring Homecare Licensed PracticalNurses (LPN / LVN) - full-time , part-time , andper diem effective immediately. We care for patients of all ages andbackgrounds who may require daily visits in their homes in the surroundingareas :

Area : Duxury, MA

Licensed PracticalNurses (LPN / LVN) Responsibilities

Assess patient's condition during every visit and document your observations

Administer medication as prescribed by the patient's Physicians perform wound care, cathetar change, disease management

Coordinate with Physicians and anyone else involved with the patient's care plan

Accurately document observations, interventions, and evaluations pertaining to client care management
