Signature Healthcare is hiring a Per Diem Physical Therapist for Rehab Services in Brockton, MA.
POSITION SUMMARY :
Under the Supervision of the Manager of Rehabilitation Services, evaluates and provides direct patient care per physician orders to rehabilitate patients in an outpatient setting to achieve stated goals. Services are provided according to hospital and departmental policies and procedures in compliance with regulatory standards.
Physical Therapist Key Responsibilities :
Completes a full initial evaluation on each patient referred appropriate to the patients diagnosis according to established procedure and physician order.Provides timely documentation at the time of evaluationProvides clear daily and progress reportingInstructs and assesses patients in a variety of functional activities.Performs and assists patients through a variety of therapeutic modalities and techniques per established procedures, utilizing techniques specific to the age group serviced.Reassess and recommend modification of the treatment plan and goals. Responds to changes in the patient's physical and mental status and any duration in the anticipated rate of progress.Meets with the members of the patients care team in development and implementation of assessments, care plans and discharge planning.Instruct patient care staff on therapeutic techniques and equipment to facilitate the carryover of the therapy plans in daily activities.Delegates treatments to PTAs to improve flow and productivityProvides ongoing patient, family, and caregiver education.Presents with a positive attitude and demeanor at all times, adheres to the WE CARE standards.Proficient in the use of computerized documentationUtilizes appropriate outcome measuresParticipates in departmental Lean activitiesParticipates in (1) departmental projects / initiatives / year.Provides supervision to Physical Therapy Assistants and rehabilitation aides.Provides clinical supervision to affiliated students under guidelines established by the participating educational institutions.May participate in the mentoring program.Establishes daily schedule, treatment plans, and priorities to service assigned patients.Maintain established policies and procedures, quality assurance, safety, and infection control.Commits to recognizing and respecting cultural diversity for all customers (internal and external).Communicates effectively with internal and external customers with respect to differences in cultures, values, beliefs and ages, utilizing interpreters when needed.Performs other duties as assignedREQUIRED KNOWLEDGE & SKILLS :
Generally works independently within department policies, however, will consult supervisor / manager regarding unusual problems or cases.Follows established policies and procedures, quality assurance, safety and infection control.Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.EDUCATION / EXPERIENCE / LICENSURE / TECHNICAL / OTHER :
Education : Bachelor of Science in Physical Therapy Experience (Type & Length) : 5 years of clinical experience or Master of Science in Physical Therapy / DPT. Experience (Type & Length) : Previous experience is not required outside clinical requirements.Experience : Requires a minimum of one year on the job to be proficient in duties. Basic CPR training will be provided.I. Certification / Licensure : Registration / Licensing in the State of Massachusetts is required.
