their communities. We are looking for a compassionate, skilled, and motivated Independent
Living Specialist to join our team and assist our clients in achieving their personal goals.
Job Overview: The Independent Living Specialist (ILS) provides direct support to individuals
with disabilities to help them live independently in the community. The ILS works closely with
clients to assess their needs, develop personalized plans, and offer ongoing guidance and
assistance to improve daily living skills, enhance social integration, and promote independence.
Key Responsibilities:
Client Support & Advocacy: Work directly with clients to assess their needs and goals,
provide guidance, and advocate for necessary resources and services.
Develop Individualized Plans: Collaborate with clients to create personalized
Independent Living Plans (ILPs) based on their unique needs and preferences.
Skills Training: Assist clients in developing essential life skills such as budgeting,
cooking, personal care, transportation, and household management.
Community Integration: Help clients access community resources and participate in
social, recreational, and employment opportunities.
Case Management: Maintain thorough records of client progress, document services
provided, and monitor outcomes.
Ensure compliance with agency policies and funding requirements.
Collaboration: Work with family members, healthcare providers, and other service
providers to ensure holistic care and appropriate support for each client.
Crisis Intervention: Provide support during emergencies or challenging situations,
ensuring the safety and well-being of clients.
Education & Empowerment: Educate clients and their families about available
resources, rights, and responsibilities related to independent living.
Building self-determination for people with disabilities and creating community change.
Qualifications:
Associate's degree and above in Social Work, Psychology, Rehabilitation, or a related
field (or equivalent experience).
Previous experience working with individuals with disabilities, preferably in an
independent living or case management role.
Knowledge of community resources, social services, and support programs for
individuals with disabilities.
Strong communication, problem-solving, and interpersonal skills.
Ability to work independently and as part of a team.
Compassionate, empathetic, and patient approach to client care.
Ability to maintain confidentiality and adhere to ethical standards.
Preferred Qualifications:
Experience with case management and/or developing individual plans for clients with
disabilities.
Familiarity with applicable laws and regulations (e.g., ADA, Medicaid, etc.).
Certification or training in independent living services, social work, or related fields.
Physical Requirements:
Ability to perform light physical activities, including transporting materials or assisting
clients with mobility when necessary.
Required occasional travel to client homes or community locations.
